Please note: Health insurance is a contract between you and your insurance company. This office will file insurance claims as a courtesy to our patients. If you have any questions or concerns about your insurance, please call your insurance prior to coming to office visit. If you have a co-pay or deductible, be prepared to pay it at time of service
